The most commonly used health forms include the Authorization of Medication Administration form and specific Health Care Plans (Allergy, Asthma, Seizure, Diabetes). Please keep in mind that ALL medications administered at school must have an Authorization of Medication Administration form completed in its entirety, including over-the-counter medications.
